Apply for a declaration about whether a named person is the parent of another named person: Form C63

Use this form to ask the court to determine someone's parentage and to give your summary of why you think your application is necessary.

Details

When you apply, include a copy of the birth certificate of the person whose parentage is in question (unless the court has directed otherwise).
